Thank you for responding to my recent model call advertisement.

The purpose of this form is to determine if you and your baby (and any older children) are suitable participants for the creative project I am currently working on.

Incomplete forms will not be accepted so please be as thorough in your descriptions as possible and answer every question that is relevant to the session you are interested in taking part in.

Depending on your answers we may find inspiration that takes us in a new direction and create an entirely new concept, just for you!

The Birth
